Homebound Heart Program This program offers healthcare and cardiac rehabilitation at home to persons who have recently experienced heart attack, heart surgery or heart disease. Homebound Heart is provided by registered nurses. The nurses will:
~ Assess cardiac status or monitor medical condition ~ Supervise Exercise ~ Instruct on diet and medication ~ Answer questions ~ Offer support and education for family members ~ Keep physician informed about progress and medical condition ~ Coordinate and update outpatient cardiac rehab on progress and readiness for that program
Health Maintenance Program Health maintenance is a self-pay program involving professional care and services to clients in their places of residence, allowing them to remain independent at home for as long as possible. It may consist of safe bathing one or two times per week or assistance with medications like setting up a med box. The client chooses what services would be beneficial and how often they would like the RN or Aide to visit.
